Coaches, Team Managers, Administrators & Volunteers
All coaches, team managers, administrators and volunteers must complete the SCYSA and US Club risk assessment forms.  Printout and complete the SCYSA form and email to DenaMauney@yahoo.com or fax to 843-449-4329.  The US Club form completed online.  Email or fax the confirmation number to the above email address or fax number.

If you have registered your child online you do not have to manually complete the required Medical/Liability Release.   You can now printout the completed form from the online registration system in two ways:  (1) When register your child for the new season and you check out you will be asked to printout the release.  (2) If you forget to print it, then go back into your registration and click on "Print Rls" beside player’s name under the heading "Players".  Be sure to update any changes to your household information such as address change, medical conditions, etc.   You must sign the release, have your signature notarized with raised seal or stamp of notary and forward the original to Dena Mauney, 59 Willowbend Dr, Murrells Inlet SC 29576.  Registration is not complete until Dena is in receipt of your Medical Release.
