2010/2011 Select Team Rosters & Registration Instructions
Dear Parents and Players,
The select team rosters for U-09 through U-18 are now posted. See rosters below.
To accept your invitation and position on the roster, you must IMMEDIATELY register online. Use the following step by step instructions to secure your position now: All players will be fitted at Soccer Locker for their uniforms in early July. Call Heyward at 843-602-8086 with any questions.
INSTRUCTIONS TO REGISTER FOR FALL 2010/2011 SEASON
Answer all questions. You will be automatically charged $150 for the uniform kit at checkout. The club has a new uniform kit starting the 2010/2011 seasonal year. All players MUST puchase a new uniform kit.
After you pay your initial payment or your entire fee, a screen will appear that says:
“Thank you! You are now registered with Coast FA."
You must click on the blue "PRINT MEDICAL RELEASE."This form must be signed, notarized with stamp or raised seal of notary, and sent with a copy of your childs birth certificate to Dena Mauney, 59 Willowhead Drive, Murrells Inlet SC 29576. We require a new medical release for every player every year.
If you are a returning Coast FA player from a previous season, you do not need to send a copy of your birth certificate.
Dena will process all players paperwork and order a player card . No player card will be released with past due balances.
If you require financial assistance, you must complete a financial assistance request. Return to the Coast FA homepage (www.CoastFA.com). Click on "Forms" under "Site Contents." Then click on the "Financial Assistance Application" link under "Scholarships" on the page that appears. Complete the form and mail to Coast FA, 551 Mount Gilead Rd, Murrells Inlet SC 29576 no later than June 30,2010.
