All McDonalds Rec League parent coaches must submit a US Club Soccer Risk Assessment application. All club administrators, select coaches and select team managers, PDC & Academy coaches must also complete a US Club Soccer Risk Assessment application. The application is completed online. To access the application, click on the link below.
When completing the application, under Individual Information, you are asked to "Choose Type Membership." Click on "Club or Academy." In the dropdwon box that appears select "Coast FA (SC) - Club #346."

All club administrators, select team coaches and team managers, PDC and Academy coaches and team managers must download, complete and return the the SCYSA Employment/Volunteer Disclosure Form to Coast FA, 628 Chestnut Rd, Myrtle Beach SC 29572 or fax to 843-449-4329. Click on the appropriate link below to download and print out the form.

SCYSA and US Club Soccer must be in receipt of the appropriated completed form before a coach can be registered, shown on the appropriate roster and carded.
