If you have been invited to play on a select team, then you must complete the Registration and Liability Liability/Medical Release before you can be rostered to a team.  This form must be completed and signed in the presence of a notary.  The notary must include his or her stamp/seal on the form.  This form must be turned in to the club.  Please mail to:

Also, if you are a new player or playing on a different select team from last season you must mail us a copy of your birth certificate along with the Medical Release.
